Do be thoughtful, this is not a "check the box" moment.  

Communication is key - choose a document that fits your wishes and that you will be comfortable discussing with loved ones and health providers.

Advance care planning documents are readily available online, see the resources  below.  You are not required to use one provided by a healthcare organization. Any valid directive will be recognized by your  health care provider. 

• Choose one valid in your state of residence.
• Find one that reflects your wishes in a future medical crisis where you are in a grave condition and unable to speak for yourself. 
• Engage the person you want to speak for you - are they able to reflect back your wishes and willing to advocate with providers in any medical setting?
• Create a valid document with proper witnessing of your signature. 
• Distribute copies to your doctor and loved ones.
