You can empower the right people to advocate for the best possible outcomes in difficult circumstances due to an accident or illness. Increase your comfort with the "what ifs" of life with quality advance care planning. My services include:

• Individual services available via Telephone, Video conferencing, Local Cafe meetings or a Home visit.
• Stimulating and thought provoking discussion to explore your wishes in the context of any health conditions. 
• Facilitation to engage your chosen healthcare agent and support the conversation with your healthcare agent to understanding your wishes.
• Training for Assisted Living Facilities and Medical Offices for internal quality assurance for resident/patient AHCDs and POLSTs.
• Public Speaking engagements are free for local Non-Profits, service groups, community gatherings, resident groups of assisted living facilities.

My process will help you create a high quality health directive:

• Contemplating what treatments or outcomes you would consider reasonable or possibly unacceptable in case of a future medical event or significant illness. 
• Connecting with a decision-maker and alternates that support your values to represent you if you are unable to speak for yourself. 
• Completing a legally valid document to advocate for those wishes that identifies a health care agent (when possible) that is accessible for loved ones and health providers.

Services and rates are structured by level of engagement and shared during your free consultation, so pricing can be matched to your specific needs. 

• Initial phone consultation of up to 30 minutes free
• Hourly rate for work with individuals 
• 3 hour Package for Individual Health Directive
• Buddy up to explore advance care planning with friends, family or your community group  - up to 6 people up to 2 hours 
• Operational coaching for Medical Office Staff, ALF/RCFE & Other Provider Training.

Some situations call for more focused conversations. These are areas I can help you think through as part of your planning:

• POLST (Physician Orders for Life Sustaining Treatment) forms - the When, the Why and the Why Not!
• Serious illness planning — updating your directive as a new or worsening health condition changes what matters most.
• Decision-maker support — preview and review how to respond when called upon to act on someones’s behalf. 
• Understanding the care continuum — geriatric and palliative care, hospice, and California's End of Life Option Act is medical aid in dying (MAID).

Services are non-refundable and not covered by Insurance.  Payment is expected at time of service.

I do not provide therapeutic or clinical counseling, health care, physical caregiving or legal/financial advice. However, in the course of services I may help identify such resources for your consideration if indicated.
