• Training for Assisted Living Facilities and Medical Offices for internal quality assurance for resident/patient AHCDs and POLSTs.
• Public Speaking engagements are free for local Non-Profits.

• Initial phone consultation of up to 30 minutes free. 
• $130.00 hourly rate for work with individuals
• 3 hour Package for Individual Health Directive is $300.00
• Gatherings for up to 6 people up to 2 hours $300.00
• $160.00 hourly rate for Medical Office Staff, ALF/RCFE & Other Provider Training, $300.00 Minimum.
• Add-on rates apply for home visits, meetings with loved ones/health care agent(s)/medical providers 

Services are non-refundable and not covered by Insurance.  Payment is expected at time of service.

My process will help you create a high quality health directive:

• Contemplating what treatments or outcomes you would consider reasonable or possibly unacceptable in case of a future medical event or significant illness. 
• Connecting with a decision-maker and alternates that support your values to represent you if you are unable to speak for yourself. 
• Completing a legally valid document to advocate for those wishes that identifies a health care agent (when possible) that is accessible for loved ones and health providers. 

I welcome invitations to provide education to non-profits, service groups, medical practices, assisted living facilities, direct service providers and other entities.
